This decision primarily is based on your personal preference because both types of implants are FDA approved, though patients must be at least 22 years old to choose silicone implants. The latest generation of silicone implants is filled with cohesive silicone gel that cannot leak if the implant ruptures. Patients often say that these implants feel more like natural breast tissue than saline-filled implants. Saline implants are filled after placement, and therefore require a smaller incision and allow for greater flexibility in sizing.

Incision & Implant Placement

When determining where to place the breast augmentation incision, Dr. Schuster considers your desires and the implant type being used. He typically places the incisions either around the areola, under the breast, or in the armpit, because these locations result in minimal scarring for most patients. Silicone implants usually require larger incisions than saline implants.

The implants can be placed either between the breast tissue and the chest muscle (subglandular) or below breast tissue and the chest muscle (subpectoral placement). The most common placement is subpectoral because it can hide the implant more effectively and has also been show to reduce the incidence of capsular contracture (a hardening of scar tissue around the implant).

Are silicone implants really safe?

After 14 years of study—making silicone breast implants some of the most studied of all medical devices— in 2006 the US Food and Drug Administration approved a new generation of silicone gel implants for women age 22 and older. Silicone has many medical uses, including a variety of applications inside the body, and silicone gel implants are considered very safe.

Will the implants rupture?

There's no guarantee that your breast implants will last a lifetime. Saline breast implants occasionally rupture and deflate, with the solution harmlessly absorbed by the body. While it's possible for cohesive silicone gel implants to rupture, the filling is likely to remain within the breast capsule. This is why regular MRIs are recommended for women with silicone gel implants – to detect "silent ruptures."
